General Instructions for Using Online PDF Forms

  • All forms are provided in Portable Document Format (PDF) and REQUIRE that you have Adobe Reader downloaded and installed on your computer in order to use them. Download Adobe Reader.
  • If you wish to save a form to your computer for viewing later, right-click on the form link and choose "Save As..." or "Save Target As..." on the menu that appears.
  • IMPORTANT: You can fill many of these forms out electronically; however, YOU CANNOT SAVE THE FORMS WITH THE DATA YOU'VE INPUT. The free version of Acrobat Reader only allows you to save a blank form. Thus, you MUST make sure you print your form with the data BEFORE you close it, or you will lose all your work and have to start over. This is a limitation built into the Acrobat Reader software on purpose, and we cannot change it.

Common Questions

I can't get the forms to come up!

Make sure you have installed Acrobat Reader on your computer. Just downloading the installation file is not enough; you must double-click it to run the installer before you can view PDF files. Oftentimes, you must reboot your computer after running the installation program as well.

I can view the forms, but they do not print correctly.

Most printing problems are the result of using an old version of Acrobat Reader. If you have problems printing, please download the latest version of Adobe Reader, install it, and try again. The current version of Acrobat Reader (as of Nov 26, 2007) is 8.1.1.

Why do you use Portable Document Format (PDF) for your forms?

Using PDF format for our forms enables everyone to have the opportunity to view the forms as they were designed. Users do not need to purchase special software; the Acrobat Reader software is available to everyone as a free download. In addition, PDF documents are readable on both Windows and Macintosh platforms. For more information on the advantages of using PDF documents, please visit Adobe's web site.
